<image:title>Healthy Children</image:title>
<image:caption>Strategies The South Carolina Early Childhood Common Agenda The Early Childhood common agenda focuses on three areas of improvement: quality of early care and education, stronger supports, and investment and accountability; all of which will contribute to the achievement of the SCHCC Goal Improve the health of children and foster the conditions to enable future healthy decisions. Annual Reports Joint Citizens and Legislative Committee on Children These annual reports analyze the situation of children in the state and each year focus on a diverse set of issues such as adverse childhood experiences, prevention of toxic stress, and early childhood language and literacy. The annual report also provides updates on the immunizations, safe sleep, obesity, trauma-informed practices, and response to sexual abuse initiatives. Top 10 opportunities for alignment The April 28th SCHCC meeting focused on the healthy development of children. A one-page summary of the top opportunities for alignment proposed by the group is available here. Where to start? Look at the maps of well-child visits by ZCTA and reading at grade level in third grade. Identify a hot-spot in your area. Leverage relationships and become familiar with existing assets in this hot-spot area (a non-exhaustive list of assets available here and in the April 28 presentation). Select one or more of the strategies mentioned in this summary.</image:caption>

Learn more Consequences of Toxic Stress During Early Childhood The growing body of scientific evidence about the power of severe childhood stress and its ability to weaken the brain’s architecture and damage lifelong health is prompting leading pediatricians to call for a seismic shift in pediatric primary care. The American Academy of Pediatrics (AAP), which represents 60,000 physicians, is planning a comprehensive public health strategy to identify and reduce toxic stress in their youngest patients. This is seen not only as a way to improve their patients’ health across the lifespan, but also as a means of improving the nation’s health—and economy. Impact of Early Adversity on Child Development Early and repeated stressful experiences have the ability to modify the architecture of the developing brain, and influence mental and physical health. Adverse experiences in early childhood such as poverty, exposure to violence, abuse or neglect are linked, among others, to depression, heart disease, diabetes and alcoholism in adulthood. Early intervention can decrease or reverse the impact of toxic stress.</image:caption>

Click above to learn about their pre-conception health initiatives. Family Solutions of the Low Country: Low-Country Healthy Start In South Carolina today, the Low Country Healthy Start program is working in high risk communities to reduce the devastating and costly consequences of infants born too soon. These infants born too early weigh too little and are very likely to continue to live their lives with less than optimum health. The Low Country Healthy Start program is focused on reducing infant mortality and eliminating perinatal disparities. Nurse Family Partnership SC Through Nurse-Family Partnership, women pregnant with their first child are paired with a registered nurse trained to educate, encourage and support them through the first two years of motherhood. This approach has proven to counter risks to babies such as low birth weight, premature birth and developmental problems. Click above to see the locations of NFP in South Carolina. South Carolina Maternal, Infant, and Early Childhood Home Visiting This program provides voluntary, evidence-based home visiting services to at-risk pregnant women and parents with young children. SC Campaign to Prevent Teen Pregnancy Teen pregnancy prevention programs are one of the best investments we can make in South Carolina’s future. According to a 2010 report by the University of Iowa’s Public Policy Center, each tax dollar spent on programs to prevent unintended pregnancy saved taxpayers an average of $3.78 in the first year—nearly a 400% return on investment. Access to Long Active Reversible Contraception (LARCs) Providing women with easy access to LARC methods, including immediately post-partum, greatly reduces the risk of unplanned pregnancies, and improves the health of newborns and mothers by facilitating healthy spacing between pregnancies. Click here for Billing policy WIC Program WIC is a nutrition program that provides health education, healthy foods, breastfeeding support, and other services free of charge to South Carolina families who qualify. To apply for WIC or to make an appointment, call 1-800-868-0404. Or locate a clinic here: http://www.scdhec.gov/Health/PublicHealthClinics/LocationsHoursContactInfo/ Centering Pregnancy Centering Pregnancy provides a fun, engaged way for women to receive prenatal care and has proven to be effective to reduce pre-term birth and low-birthweight. Mothers’ Milk Bank of South Carolina The Bank promotes the health of South Carolina babies by providing access to safe, pasteurized donor human milk to very low birth weight (VLBW) babies. Human breast milk prevents Necrotizing Enterocolitis (NEC) which is a life-threatening infection typically affecting babies born before 32 weeks gestation. Databases and Tools South Carolina Community Assessment Network (SCAN) This is the DHEC tool that allows organizations to access information about births, deaths, cancer incidence, infant mortality, hospital acquired infections, population, pregnancy, and community profiles. Bright Spots outside of SC Pathways model decreases by 50%+ Low Birth-weight in at-risk mothers Pathways is a model that identifies women who are at risk of having poor birth outcomes, and uses community health workers (CHWs) to address the women’s health and social service issues. <image:caption>Coalitions Birth Outcomes Initiative The South Carolina Birth Outcomes Initiative is an effort of over 100 stakeholders to improve the health outcomes for newborns in South Carolina. Their primary focus areas are: Elimination of elective inductions for non-medically indicated deliveries prior to 39 weeks gestation Reducing the number of admissions and the average length of stay in neonatal intensive care units Reducing health disparities Making 17P, a compound that helps prevent pre-term births, available to all at-risk pregnant women with no “hassle factor” Implementing a universal screening and referral tool (SBIRT) in the physician’s office to screen pregnant women and 12 months post-delivery for tobacco use, substance abuse, alcohol, depression and domestic violence Promoting Baby Friendly Certified Hospitals and Breast Feeding For more information on South Carolina’s Birth Outcomes Initiative, contact BZ Giese Safe Sleep Coalition At the request of the Joint Citizens and Legislative Committee on Children, Children’s Trust of South Carolina formed the statewide South Carolina Safe Sleep Coalition in January 2012. The Coalition represents more than 30 organizations convening to address rising rates of unsafe sleep practices that result in injury and death. Including hospital systems, public social service agencies, nonprofit organizations and state agencies, the members collaborate to positively affect the following statistics: South Carolina is ranked 45th in Nation for Infant Mortality. Sudden Infant Death Syndrome (SIDS) continues to be third leading cause of death in infants. Despite declining overall infant mortality rates in South Carolina, deaths due to SIDS increased by 11.4 percent in 2010. The number of infant deaths in South Carolina due to accidental suffocation and strangulation in bed has more than DOUBLED since 2004. The Coalition’s focus is to deliver a single safe sleep message to parents, caregivers and providers through health, child care, faith-based, social services, community and public safety organizations. On March 2014, the first Safe Sleep Summit brought together more than 150 individuals from diverse agencies who helped develop specific strategies and practice recommendations to reduce the number of infant deaths in South Carolina from unsafe sleep environments. Additionally, a safe sleep curriculum and implementation plan is currently being developed to share with partners. <image:title>Our Approach</image:title>
<image:caption>Collective Impact Collective Impact is a framework that resulted from the understanding that no single organization has the ability to solve any major social problem at scale by itself. When it comes to health in South Carolina, there’s a lot of room for improvement. Our costs are high, but our health is poor—and because of economic, racial and geographic disparities, it’s hard for many children or adults in the Palmetto state to live healthy lives. This is why the Alliance has committed to joining efforts with all others who are interested in improving the health of ALL people in South Carolina. In order to generate the Collective Impact we need to accelerate improvement, the 50+ members of the Alliance have been working with and learning from the multiple health-related coalitions in the state, to develop the building blocks of the Collective Impact Framework in our state: Common Agenda See Priorities and Call to Action for Health Equity Shared Metrics See Dashboard Mutually Reinforcing Activities See Case Studies, Briefs, and other contents of this website to join others in improving health and healthcare for ALL Share your own activities that are helping achieve our Common Agenda Continuous Communication Join our newsletter Backbone Support Catalyst for Health is currently supporting Alliance members and partners to achieve the 2020 goals The first Collective Impact article was released in 2011. <image:title>Our Approach</image:title>
<image:caption>The Triple Aim The Alliance efforts are intended to meet the tenets of the “Triple Aim”, which is a single aim, with three dimensions: Improve population health This refers to the improvement of health for ALL people in South Carolina. It includes outcomes of clinical care, and of the social, economic, and environmental conditions that help or hinder all of us from achieving our optimal potential for good health. Improve the experience of care This refers specifically to healthcare services that are safe, effective, patient-centered, timely, efficient, and equitable for ALL people in South Carolina. Lower per-capita costs of care This refers to the overall expense in healthcare in South Carolina (insurance premiums, out of pocket expenditures, charity care, etc) divided by the number of people in the state. The Institute for Healthcare Improvement has more information about the Triple Aim and term disambiguation.</image:caption>

<image:caption>Strategies The Call to Action for Health Equity focuses on changing the way organizations see their role in helping diverse populations make healthy choices, access health and social services, and enjoy healthier physical and social environments. It is composed of four actions that will help our organizations understand obstacles to health improvement and how to work together to develop sustainable solutions for accelerating health improvement for everyone in South Carolina. Data-driven interventions We can use data to discover which groups of people may need extra support from our organization and partners. Cultural competence & responsiveness We can assess and train ourselves to have more empathetic relationships with people of different backgrounds. Community Engagement We can partner with communities to increase the impact of health improvement interventions. Inclusive decision making We can invest in maximizing opportunity for diverse groups of the population to be included at all levels of decision making. Data-driven Targeted Interventions: Collect and use data to identify and guide decision making regarding the health equity challenges in SC. Collect data to identify health equity challenges experienced by our employees and clients and use it to design and implement targeted interventions. Use secondary data to identify health equity challenges in the geographic areas of influence of our organizations and use it to design and implement targeted interventions. Cultural Competence and Responsiveness: Develop and maintain a culturally competent and responsive organizational culture for employees, clients and partners. Inclusive Decision-Making: Ensure our organizations are designed to guarantee inclusive decision making. This can be achieved through enhanced emphasis on the recruitment of highly qualified minorities for leadership positions, and the investment of filling the pipeline with diverse leaders for this and future generations. Community Engagement: Engage the community as partners in the design and delivery of sustainable health solutions. Read more about the South Carolina Call to Action for Health Equity here.
